What are the costs for services at Clover Physiotherapy?
The cost for physiotherapy services are as follows:
Initial Assessment (60 min) $125
Physiotherapy 30-minute follow up $85
Physiotherapy 45-minute follow up $110
Physiotherapy 60-minute follow up $135
Do you direct bill to insurance?
Yes! We direct bill to most major insurance companies. Please make sure to bring your benefits card and a piece of ID to your first appointment to ensure a smooth billing process.
We do accept MVA insurance claims.
We do not currently accept WSIB insurance. We do not bill to OHIP.
Do I need a doctors referral for physiotherapy?
At Clover Physiotherapy we do not require a referral from a doctor to access physiotherapy services. However, some insurance companies may require a referral if you want to use benefits, so it is best to check with your insurance company ahead of time.
How should I prepare for my visit?
- You will be sent intake and consent forms prior to your visit, so ensure you have these filled out before arriving at the clinic. Alternatively, you can fill out paper copies of the forms when you arrive at the clinic.
- Please arrive 5-10 minutes before your scheduled appointment time.
- Bring your benefits card and a piece of ID if you would like us to direct bill to your insurance company.
- Bring your referral if you were referred by your doctor.
- Be sure to dress comfortably for your appointment as you will be asked to move around a lot. For upper body issues, it is best to wear a tank top. For lower body issues, it is best to wear shorts.
